How to complete your follow-up form

This form is required to be complete prior to your appointment time. Answering these questions allows more time to discuss issues important to you. It is also part of comprehensive patient care. If the form is not complete before your appointment, we will be unable to keep your appointment time. If we have to cancel due to forms being incomplete, you will be charged $50 for the missed appointment.
We will add the form to your portal the Thursday the week before your appointment if you would like to complete in advance.

Step One: Login to your patient portal using your email and the password you created.*

*If you have forgotten your password, please reset on this site or contact the office to reset your password.

Step Two: Once logged in, click on the form buttons as indicated below.

Step Three: Click the start button to begin your form.

Step Four: Complete all questions and click submit.

Step Five: You are finished! Remember to login at least 10 minutes prior to your telemedicine appointment. If you have ANY issues do not delay in texting us 918-270-4100 for technical support. We utilize Zoom for all appointments; please download prior to your appointment.
